The areola is the pigmented area around the nipple which contains muscle and therefore contracts in the cold and with stimulation of the nipple. The areola varies enormously in its appearance just like the shape and size of the breasts. The size (diameter) and shape (round vs elliptical vs irregular) may vary but also the edge may vary too (well defined vs blurred).
We understand that the nipple and and surrounding areola is an integral part of the beauty of the breast and indeed contributes to one’s sexuality so the appearance can sometime generate untoward thoughts .Some girls enquire for consultation with Dr Paul because they are also concerned regarding the proportion of the their nipple/areolar in relation to their existing breast size but also because of where it might sit within the breast. The position of the nipple (and the areola) on the breast will also determine the complexity of any surgical solution
The critical point here is that there is a huge variation of areolar anatomy and morphological types and Dr Paul will discuss your unique anatomy with you. Whilst simple areolar reduction can easily be performed in some cases under a local anaesthetic this is NOT the norm. Sometimes we have to have more complex discussions with you and explain that an areolar reduction alone may not be recommended or feasible. In girls with stretchy skin, crepiness, a large areola or a low nipple (or a combination of these features) then often a mastopexy (breast uplift) might be more in keeping and recommended by Dr Paul. Rest assured Dr Paul will give you his expert advice but often this news is not always expected (or wanted) so managing expectations with this operation is a crucial part of the process. Scar stretching and further areolar stretch is always a consideration so this needs to be discussed in an open transparent way.

Areola reduction surgery is performed by removing a circular area of the outer brown area of the areola. The skin surrounding this is then brought inwards in a purse string fashion to surround the areola. Absorbable stitches inside the areola are used to reduce the diameter and, in most cases, a permanent stitch placed deeply in the tissues to prevent widening of the areola occurring following surgery. To begin with, it is usual to have ruckling of the skin but this will settle over time and accelerated by regular massage of the scars. By using the permanent stitch it is very unlikely that you would get further widening of the areola and your nipples and the areola should stay in a smaller position.
The benefits of areola reduction are a permanent reduction in the size of the areola with scarring limited to the junction of the brown skin and the surrounding normal skin of the nipple.
The procedure can be performed under either local or general anaesthetic as a day case, has a minimum down time and also has the other advantage of tightening up the breast skin.
You will be able to return to sedentary activities (i.e. an office job or light duties) after a couple of days. Heavy physical activities and contact sports can be resumed following your post-surgery appointment with Mr Banwell.
These operations have a high success rate, but, as everyone is different and the treatment plan will be individualized to every patient, the likely outcome you will obtain will be discussed directly with you.
